Meg has launched this option for people who want a prescribing functional medicine practitioner, to manage current conditions and prescriptions, but who don’t want to spend a lot of money on appointments or time waiting to be seen.

This option is best for those who need routine check-ups on a current condition, and want the freedom to be able to request bloodwork when needed (e.g. for thyroid hormone checks), and aren’t needing a lot of care or evaluation but would like to be able to ask questions when needed.

The following stabilized conditions are best for this model:

  • Hypothyroidism or Subclinical Hypothyroidism
  • Non-insulin dependent Type 2 Diabetes and/or metabolic syndrome
  • High cholesterol
  • Those who want to prevent familial diseases (diabetes, heart disease, autoimmune disease, Alzheimer’s Disease, etc), and want to optimize preventative testing.

If more care is required, Meg typically recommends 2-3 visits to get started on a protocol, then transition to monthly membership as health improves.  If this occurs, the registration fee is waived.

*Of note, HSA and FSA may cover the cost for membership if paid in full.  

**If a patient has insurance, lab companies will bill to insurance.  If no insurance, we work with a lab that provides great cash prices.

  • Membership fees:  
    • One-Time Registration Fee (not required for yearly renewals): $399
    • Monthly membership fee: $85 *
      • Includes 2-60 minute visits per year 
        • No carry-over visits
      • Includes secure email communication via the patient portal (per email guidelines)
      • Includes short interpretation of conventional test results (5-10 minute interpretation)
        • Examples include follow up thyroid hormone checks, follow-up tests for bioidentical hormone replacement, food intolerance testing, flu testing, strep testing, urine testing for UTIs.

  • Cost outside of the scope of agreement
    • If more care is needed beyond the included 2 visits, patients will be responsible for payment of follow up visits at the time of service, per the established patient prices.
  • Change in fees
    • The membership fee may be changed by CCM.  If patients paid for a membership in full for one year, this will not impact cost until renewal.  If the new fee is unacceptable, patients are not obligated to renew membership.
  • Registration fee includes in-depth 75 minute evaluation in the office for functional exam, personalized treatment recommendations. 
  • Membership includes 2 follow up visits per year to evaluate progress and adjust treatment plan 
  • Includes brief 5-10 minute phone visits for on-the-go convenience.  Option for same-day scheduling (if scheduled before noon) to get questions answered, testing ordered, etc

    • If lab results require an extensive discussion of lifestyle recommendations or a personalized protocol, a follow up appointment (per established patient pricing of $125-$325) will be recommended 
  • Secure email (via patient portal) communication with practitioners and CCM staff.  Occasional phone calls may also be available, if indicated.  Patients are asked to review the email guidelines and ensure to the best of his/her ability that emails will maintain the parameters set forth by the email guidelines.
  • Additional visits (beyond the 2 included appointments):
    • 15-20 minute appointment: $125
    • < 1 hour appointment: $225
    • > 60 minute appointment: $325
  • This membership does not provide 24/7 access to practitioners, and there may be times when the practitioner is not available due to patient care, personal illness/injury, emergencies, or other obligations.  Although it is the goal to provide same day or next day services during business hours (i.e. no weekend responses), there are circumstances where this service may not be available. 
    • If a medical emergency arises, patients are instructed to call 911 or visit the nearest hospital emergency room.
  • Cost associated with lab testing: Includes (but are not limited to):
    • X-rays, CT scans, ultrasounds, and other medical imaging
    • Swabs for flu or strep testing
    • Urinalysis to check for urinary tract infections
    • Blood tests
    • Functional medicine testing (OAT, stool tests, saliva tests, etc)
    • Please note that insurance can be billed for most of the above; insurance determines the extent of coverage (partial, full, or no coverage)
  • Cost for supplements (CCM uses an online dispensary, and offers all members 20% off any supplements ordered via this company)
  • Cost for appointments that require care of a new health condition or that require a new treatment (pharmaceutical or therapeutic supplement, therapeutic diet/lifestyle plan)
  • Acute care (e.g. musculoskeletal injuries, lacerations, antibiotics or antivirals for untested conditions, etc)
    • We can order tests for flu, strep, and UTI screening 
  • Obstetrical and gynecological care (no gynecological or hernia exams are included for any appointment.  CCM expects all patients to have access to a healthcare provider that can provide these services when needed.)
  • Pediatric care (to those under age 10)
  • Cost for prescription medications 
  • Cost for injectable medications (including immunizations, which we do not provide in office)
  • The healthcare practitioner will provide a limited set of medical and health services in exchange for a membership fee, services which are provided as long as patient pays the monthly membership fee (or has paid for an entire year up front). 
  • Refunds: Refunds for membership will be at the discretion of Center for Collaborative Medicine, dependent on amount of care that ensued over the duration of limited membership. 
    • Registration fees are non-refundable.  
    • Termination of membership with subsequent refund will factor in the number of visits completed.
    • We are a reasonable company, and understand that sometimes circumstances present that affect ability to cover costs for services.
      • We do not provide financing plans, but we will do our best to work with members who must terminate membership, commensurate with the amount of care a patient has received.
  • Renewal: To continue an existing membership, patients have the option for automatic monthly withdrawal.  If patients discontinue membership, renewal is available if done within 6 months from date of membership termination.  After 6 months, patients will be required to pay an additional registration fee to become a member again.  
  • Center for Collaborative Medicine provides functional and integrative medicine services, and does not function as a primary care or family medicine practice. 
  • Meg McElroy MS, PA-C will be the lead provider for members.
  • Our focus is on:
    • Less green allopathy (i.e. avoiding over-supplementation to “treat” symptoms and lab results).
    • Management of thyroid conditions, metabolic conditions (e.g. non-insulin dependent diabetes, cardiovascular risks), hormone balance (particularly for those that are peri- or post-menopausal), and more.  Management includes current prescriptions.  New prescriptions can be obtained after medical visit with provider.
    • Over the 20+ years we’ve been involved in functional/integrative medicine, we’ve seen that lifestyle habits and daily environmental exposures determine health trajectory (vs treating test results with pills and procedures).  Health coaches are great resources for those that need more accountability and guidance on optimizing daily habits (we can refer patients to health coaches if desired).
  • Patients are advised to have a relationship with a primary medical provider separate from Center for Collaborative Medicine.
